P3B238: Polyarticular juvenile Idiopathic Arthritis: Building a System of Care to Improve Patient Compliance and Provider Connections

Background: Polyarticular juvenile idiopathic arthritis (pJIA) impacts 300K children in the US and is the most common chronic childhood rheumatologic disorder. There is a nationwide shortage of pediatric rheumatologists. Due to this, the primary care provider (PCP) plays a pivotal role in the recognition and management of pJIA in the community based, patient centered medical home.
Early recognition of pJIA and referral to specialist by the PCP dramatically impacts outcome. Familiarity of unique health maintenance needs of the child with pJIA improves care.

Methods: The Ohio Chapter, American Academy of Pediatrics (Ohio AAP) aimed to establish a system of care to increase knowledge, confidence, and competence of PCPs in diagnosing and managing patients with pJIA in the community based, patient centered medical home through education and resource development.
In preparation of new material creation for PCPs, patients/caregivers and for rheumatologists, existing research, resources, evidence-based practices, and educational activities were examined.
4 webinars were created with education on screening and diagnosis, management of active symptoms and chronic pain, mental health impacts,
co-management with rheumatology, and when to refer.
An asynchronous Journal Club was created; 10 articles on expanded topics were selected.
All education and materials were housed on OhioAAP.org.

Results: Immediate impact on participant knowledge and intent to change was measured after the course via evaluation.
70 providers registered to attend the CME activities.
3 physicians completed Journal Club.
40% of the 16 evaluation respondents committed to changing practice.
86% said the content matched their scope of practice.
98% said the speaker(s) were effective.
96% were satisfied with their learning activity.

Conclusion: Through the creation of educational materials, virtual webinars, and an online Journal Club, we were able to share evidenced-based tools for providers and aid in changing practice.
NEXT STEPS:
Digital education and resources will be available on the Ohio AAP website with planned spread in 2023. We would also like to look at measuring the impact on clinical outcomes.
